1 All rates are subject to change by C&W USA. Rates only apply if services are purchased as Bundled Packages as set forth above.
2 Long distance voice rates are subject to C&W USA's standard call cost rounding (i.e., the charge for a call is rounded up to the next full cent) and standard billing increments/minimum per call charge. Customer must bill at least $50.00 per month in usage charges. If Customer's usage charges are less than $50.00 in a month, C&W USA will bill the customer for its actual usage that month plus the difference between the customer's actual usage charges that month and the $50.00 monthly usage minimum.
3 This rate includes 150 hours of local access Internet usage each month. C&W USA shall bill customer $.99 for each hour over 150 hours per month and $5.95 per hour for each hour of Toll Free or ISDN access.
4 Customer is required to purchase a router for this service. The current price for a router is $415.00.
5 The Monthly Port rate does not include the local loop.
